What is Collaborative Care?

Collaborative Care is a proven, team-based model where your primary care provider, behavioral health therapist, and a psychiatric consultant work together to ensure you receive the best possible care—all connected under one plan. This approach is especially helpful for individuals managing depression, anxiety, chronic stress, or complex medical and mental health needs.

Instead of juggling multiple providers on your own, your care team works behind the scenes to coordinate your treatment, monitor your progress, and adjust your plan as needed—all while keeping you at the center.

How It Works: Meet Your Team

Our three-pronged approach connects you to a team that supports every part of your health journey:

Amur Medical (Primary Care)

  • Screens for both physical and mental health concerns.

  • Manages primary care needs and prescribes medications with guidance from our psychiatric consultant.

TLC Counseling Services (Behavioral Health & Care Management)

  • Provides therapy, care management, and ongoing support.

  • Tracks your progress and ensures all providers are aligned in your care plan.

  • Coordinates communication between your primary care provider and psychiatric consultant.

Psychiatric Consultation

  • Offers expert psychiatric consultation.

  • Guides diagnosis and medication recommendations to support your care plan.

  • Does not prescribe directly but works closely with the PCP to inform treatment decisions.

Together, we ensure your care is comprehensive, connected, and compassionate.

  • Initial Screening: Your journey begins at Amur Medical or TLC Counseling with a simple screening to identify your physical and mental health needs.

  • Warm Handoff & Consent: If Collaborative Care is a good fit, we’ll introduce the team, explain the process, and ask for your consent to share information across providers.

  • Behavioral Health Assessment: You’ll meet with a Behavioral Health Care Manager at TLC Counseling to dive deeper into your mental health needs.

  • Care Planning: Your care team—including your PCP, therapist, and psychiatric consultant—reviews your case and builds a personalized plan.

  • Medication Management: If medication is recommended, your PCP at Amur Medical prescribes it with insights from the psychiatric consultant.

  • Ongoing Monitoring & Support: Your Behavioral Health Care Manager tracks your progress, provides therapy, and ensures the team adjusts your plan as needed.

  • Review & Adjustments: Weekly team reviews help ensure your care stays on track and evolves with your needs.

  • No. If you already have a primary care provider, we can work with them. If not, we’ll connect you with Amur Medical, one of our trusted partners.

  • Your primary care provider at Amur Medical manages all prescriptions, including psychiatric medications. These decisions are made in consultation with our psychiatric consultant to ensure your treatment is safe and effective.

  • Yes! Most Medicaid plans cover Collaborative Care services. We’ll verify your benefits and explain any costs before starting.

  • We’re happy to coordinate with your existing therapist. Our Behavioral Health Care Managers focus on care coordination and symptom tracking but can work alongside your current provider.
